Title: Piecework Targeted Case Managers
Reports To: Sharane Knibbs, Program Coordinator – Case Management Program (TCM,
Location: South Region
Responsible for coordinating services for individuals and families who qualify for case management services as defined by Medicaid. Services include but are not limited to: Medical, social, legal, educational and other support services. Will work with SED children and their families. Must be willing to travel throughout Miami-Dade County. Maintains appropriate documentation for each client; evaluates client’s home and school environment and interaction; arranges treatment services for clients as per treatment plan; acts as liaison to special projects.
Must have a Bachelor’s degree from an accredited college or university in social work, psychology or a related field and have a minimum of one year of full time or equivalent experience working with children with serious emotional disturbances; or have a Bachelor’s degree from an accredited college or university (non-related field) and three years full time or equivalent experience with children with serious emotional disturbances. Community development experience and knowledge of available resources in the service area for children with serious emotional disturbances preferred. Bilingual (English/Spanish) a plus.
